Sports Medicine
The Anderson University Sports Medicine Department strives to provide the best Sports Medicine Care possible for student athletes participating in intercollegiate athletics at Anderson University. One part of this goal is obtaining up-to-date information about each student athlete.
In order for us to create and/or maintain a file on you, we need you to print off a copy of each form and return them to us by August 1, 2008. If you are having difficulty obtaining the forms, please feel free to email rneault@andersonuniversity.edu or call at (864)-231-2144.
Again, let me remind you that the above forms must be completed and returned to us by August 1, 2008. No student will be permitted to participate in athletics without this information on file in the Anderson University Sports Medicine Department.
All forms must be completed in FULL. Please complete all lines and check any and/or all boxes. ("Same as Last Year" will not be acceptable.) Please send forms to the following address:

New Policy Note: All student athletes are required to complete a pre-participation physical evaluation (signed by a licensed MD or DO) each year.

- Medical Policy, Wavier of Risk Liability Form, HIPPA Release and Permission For Treatment
- Drug Screen Consent Form and First Agency
- Travel Information Form

- Preparticipation Physical Evaluation
- Any student interested in working in the Training Room needs to contact the head athletic trainer, Vanessa Rettinger
- Students who work in the Training room will work about 10-12 hours a week in the following settings:
- Training room
- Practice Coverage
- Game Coverage
- (All work will be done under the close supervision of a Certified Athletic Trainer)
- Students will need to apply for Work Study money through the Anderson University Financial Aid Office. After the financial aid office confirms the student is eligible and confirms the amount of aid, a work agreement is signed between the student and the head athletic trainer.
- The head athletic trainer will have meetings with each student once a month to evaluate the progress of the student both in the classroom and in the athletic training room
- If you are interested in pursuing a job in the Anderson University Sports Medicine Department as a Sports Medicine Assistant, please complete the following:
- A 150 word minimum short essay about your interests in Sports Medicine. (Example, i was an athlete who had a knee injury, went to therapy and enjoy helping people.) Please submit the completed essay to the head athletic trainer.
- Contact the Anderson University Financial Aid Office and complete all the necessary paperwork.
